Current Evidence for Infection Prevention and Control Interventions in Emergency Medical Services: A Scoping Review

Objectives:
The aim of this review was to summarize current evidence from the United States on the effectiveness of practices and interventions for preventing, recognizing, and controlling occupationally acquired infectious diseases in Emergency Medical Service (EMS) clinicians.
Report and Methods:PubMed, Embase, CINAHL, and SCOPUS were searched from January 1, 2006 through March 15, 2022 for studies in the United States that involved EMS clinicians and firefighters, reported on one or more workplace practices or interventions that prevented or controlled infectious diseases, and included outcome measures. Eleven (11) observational studies reported on infection prevention and control (IPC) practices providing evidence that hand hygiene, standard precautions, mandatory vaccine policies, and on-site vaccine clinics are effective. Less frequent handwashing (survey-weight adjusted odds ratio [OR] 4.20; 95% confidence interval [CI], 1.02 to 17.27) and less frequent hand hygiene after glove use (survey-weight adjusted OR 10.51; 95% CI, 2.54 to 43.45) were positively correlated with nasal colonization of Methicillin-resistant Staphylococcus aureus (MRSA). Lack of personal protective equipment (PPE) or PPE breach were correlated with higher severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) seropositivity (unadjusted risk ratio [RR] 4.2; 95% CI, 1.03 to 17.22). Workers were more likely to be vaccinated against influenza if their employer offered the vaccine (unadjusted OR 3.3; 95% CI, 1.3 to 8.3). Active, targeted education modules for H1N1 influenza were effective at increasing vaccination rates and the success of on-site vaccine clinics.
Conclusions:Evidence from the United States exists on the effectiveness of IPC practices in EMS clinicians, including hand hygiene, standard precautions, mandatory vaccine policies, and vaccine clinics. More research is needed on the effectiveness of PPE and vaccine acceptance.

Introduction
Neurologists' knowledge of number processing impairments is often limited to the notion that acalculia is part of Gerstmann's syndrome (Benton, 1992; Gerstmann, 1940). It may extend to the classical typology proposed by Hécaen, who distinguished aphasic, spatial, and anarithmetic acalculia (Hécaen et al., 1961). Actually, modern research on acalculia (a term coined by Henschen, 1920) has made considerable progress from this point, starting in the 1980s within the expanding framework of cognitive neuropsychology, and soon yielding refined cognitive models (Dehaene, 1992; Deloche and Seron, 1982; McCloskey et al., 1986). Presently, it is an important component of the cognitive neuroscience of numerical abilities, having close links to the methodology used in and theories arising from numerical cognition studies performed in animals, human infants, or normal adults. These methodologies include behavioral measures, functional and anatomical imaging, and electrophysiological techniques (for reviews see Butterworth, 1999; Dehaene and Cohen, 1995; Dehaene et al., 2004).
Beyond its fundamental interest, acalculia constitutes a frequent and incapacitating disorder following acquired, mostly left-hemispheric, brain lesions (Jackson and Warrington, 1986; Rosselli and Ardila, 1989). It interferes with many everyday life activities such as shopping, assessing the balance of a bank account, etc.
In this chapter we will summarize the basic facets of the normal number processing abilities, sketch a simple model of their anatomical implementation, describe an illustrative variety of calculation disorders, briefly discuss the relationships of acalculia, Gerstmann's syndrome and the parietal lobes, and finally propose some guidelines for the assessment and rehabilitation of acalculia in stroke patients.

The fourth season of the Fezzan Project continued the interdisciplinary approaches of previous seasons. Geographical and environmental work focused principally in sampling sediments for scientific dating and with integrating ground observation with remote sensing data. Excavations continued at Old Germa, where the site has now reached Garamantian levels. In a separate development, the tentative identification has been made of an early mosque at the site, in an area adjacent to the G1 excavation trench. Substantial results were gained from work aimed at enhancing the important data recorded by Charles Daniels in his earlier excavations and survey in the Wadi al-Hayat. The enhancement of the Daniels' survey archive was integrated with completion of the wider prospection being undertaken by the new project. This survey included fieldwalking, standing building survey, analysis of the foggara irrigation systems and recording of rock art scenes. Finds work comprised the finalisation of a pottery type series for the Germa area, the study of small finds from the recent survey work, botanical analysis and completion of lithics recording. A programme of radiocarbon dating is now being undertaken to improve the phasing of sites and monuments. The first two volumes of final reports are now in preparation.

The Fezzan Project is investigating the last 10,000 years of human settlement, landscape evolution and climatic change in the Germa region in southern Libya. The second season in February–March 1998 comprised interdisciplinary research in archaeology and geography, centred around excavation and survey work carried out at the site of Old Germa. To date, three phases of mud brick buildings have been partially explored. In addition, wider geomorphological study and archaeological survey and fieldwalking were carried out elsewhere in the Germa/Twesh oasis and around el-Hatiya. Numerous sites were discovered, including a new hillfort of Zinchecra type and several valley centre ‘villages’ of Garamantian/Roman date. Artefactual studies were carried out on pottery and lithics, animal bones and seeds. Further work on the subterranean irrigation features, the foggaras, have confirmed their pre-Islamic origins.
